Purpose

The YPA School Nurse Program serves to promote health and safety, facilitate positive student development, and to intervene with actual or potential health issues that could pose as a barrier to the education process. We are here to serve students with chronic conditions, as well as emergent needs and minor injuries that happen during the school day. Contact your school nurse if we can help in any way.

Parent Access to Immunization Portal

SC Department of Public Health (DPH) has updated the immunization portal to allow parents and legal guardians to access and print completed shot records for their students. This means you will no longer have to contact your physicians office to obtain the official DPH School Certificate of Immunization (DPH4024). Utilize this LINK (https://dph.sc.gov/public/vaccinations/online-access-immunization-records) for step-by-step instructions and reach out to your building nurse with any questions.

Contagious and Communicable Disease Tracking

York Preparatory Academy is required to track contagious and communicable diseases including but not limited to: Flu, Covid-19, respiratory, gastrointestinal, skin, and other disease categories listed on the SC Exclusion List. If your student has received a diagnosis from a physician, please use the following FORM LINK to help us identify, report, control, and prevent the spread of infectious disease.

Staying Healthy

Here is a reminder of the best health practices to decrease the spread and transmission of viruses here at school and throughout the community.

  1. Please remember to cover your mouth and nose with a tissue when you cough or sneeze and throw used tissues in the trash. If you don't have a tissue, cough, or sneeze into your elbow, not your hands.

  2. Wash hands often with soap and water for 20 seconds, dry hands with a paper towel, and use the paper towel to turn off the faucet. If soap and water are not available, hand sanitizers may also be used to perform hand hygiene.

  3. Remember to stay home when you are sick and follow the recommendations listed on the SC Exclusion List.
